At Milford Pediatric Dentistry, it is important to us to make it simple and affordable for our patients to receive the dental care they need and want. We are excited to share with you the in-house Smile Membership Plans.
The Smile Membership Plans plan is not an insurance policy and is a great value! Because we believe in preventing problems before they start, we’ve placed an emphasis on preventative care as well as offering discounts on ALL services for a low monthly or annual fee.
By utilizing our Smile Membership Plans, our goal is for our patients to save money, improve and maintain their oral health, and avoid the hassle and headache of traditional dental insurance. The prices below relate to services provided in our office by our providers.
A dental home is a community cornerstone that looks after your child’s oral health from their first tooth onward.
Our membership plans are not insurance, pre-payment for future services, payment for access to discounted services, or a qualified health plan under the Affordable Care Act. Instead, our patients elect to make a monthly or annual subscription payment for preventive care versus paying at the time of each service.
• The membership plan is a direct agreement between the patient and the legal entity of our practice. Membership plan payments must be current in order for the patient to receive the benefits included with their membership plan.
• The prices above relate to services provided in our office by our providers.
• Membership plan benefits may not be combined with any other offers or discounts. Payment for additional treatment provided by our practice to the patient is due at the time of service and our membership plans do not cover procedures completed outside of our practice.
• The term of our membership plans is 12 months from the patient Effective Date and automatically renews on the Renewal Date. Patients can opt-out of membership plan renewals by clicking “Cancel Auto-Renew” in the Member Portal or by contacting our practice directly. Refunds are at the sole discretion of our practice.
• The child’s guardian is responsible for scheduling periodic care/cleaning appointments at least 6 months ahead to ensure appointment availability twice per year.
